  • If you do not have an email address an email will automatically go the Salvation Army and your confirmation will be generated and mailed to you. PLEASE CHECK YOUR MAILBOX FOR YOUR CONFIRMATION.

You must provide current ID or document with current Name & Address

Please upload an ID with a currant address or a piece of mail with name and current address.  (If on a computer with video camera, you can use this to take a picture)

Confirmation of Appointment

You will receive a confirmation Email or postcard with your appointment Date and Time.  Please do not show up early as space is limited.  We encourage you to bring child(ren) to try on coats.

I agree to allow The Salvation Army of Oshkosh to receive from and share my information with other local Christmas agencies and churches as it pertains to Christmas assistance. I understand this release is only for Christmas assistance. I also understand that items received are based on donated items and that The Salvation Army of Oshkosh cannot guarantee I will receive exactly what I am looking for. Also, by signing this form I agree to allow The Salvation Army to enter my personal information into a secured Salvation Army Client Data Management System.
