Love Baskets-Family Registration Form

If you need help this Christmas and live in the New Richmond or Somerset School Districts, Love Baskets can help.  We are a non-profit organization, working together with The Salvation Army and Toys for Tots, committed to helping children and families at Christmas time.  Please fill out the following form as clearly as you can. To be eligible, you must:

• Be the head of the household applying for children aged 0 – 18 years-old.
• Be applying for children in Somerset or New Richmond.  
• NOT be receiving duplicate assistance from another agency, church or organization.
• AGREE to the terms and conditions at the end of this form.

Directions: 

• Please complete all of the required information to the best of your ability. 
• If two families live together in the same house, each family needs to register separately.

APPLICATION DEADLINE: November 15, 2024

You will soon be receiving a confirmation with your ID#.  It will include instructions on your pick up time, location and process.  You need to be prompt with your pick up time.  This is the only day we will be handing out gifts.  You must be present December 22nd from 1p-4p  to receive your gifts for your family.  Only those who receive a confirmation number and further instructions are CONFIRMED.
